The City of Milwaukee Health Department will hold two vaccination clinics this week as listed below. Clinics are now open to all Milwaukee citizens.
H1N1 flu vaccinations will be voluntary, free of charge, and be provided on a first-come first-served basis in accordance with the recommendations released by the State Division of Public Health (DPH). These clinics are being promoted for underinsured and uninsured individuals and their families.
WHEN: Thursday, November 19th, 2009
WHERE: Sara Scott School, 1017 N. 12th Street
TIME: 3:00 pm to 7:00 pm
and
WHEN: Friday, November 20th, 2009
WHERE: South Division High School, 1515 W. Lapham Blvd.
TIME: 3:00 pm to 7:00 pm
A clinic consent form is posted above and is available in both English and Spanish. The MHD strongly encourages residents to download and complete the form and bring it with them to the clinic to assist in quicker registration processing. To find the clinic consent form, click here.
Seasonal flu vaccines and childhood immunization will not be administered at any of the H1N1 vaccination clinics. For more information, contact 2-1-1.
