Nomination Paper Sample & Instructions
The State form, EB-169, is a sample of a nomination petition, which may be circulated in order to get the minimum required signatures for ballot placement.
If the candidate desires to make changes to this form, the amended form must be approved by the City of Milwaukee Election Commission.
Failure to circulate an approved nomination petition may result in the candidate losing ballot placement.
When the signatures are turned in, the City of Milwaukee's Election Commission staff will verify that the addresses affiliated with the signatures are within the City or the Aldermanic District boundaries. This is done on the voter registration computer system.
