Am I Eligible?

As the General Assistance Medical Program has limited funds, a set of qualification criteria has been established.


  • You must have been a Milwaukee County resident for the last 180 days.

  • You cannot be eligible or on any other entitlement program (T-19 or T-18 ) or any third party public or private insurance.

  • Social Security Number must be verifiable.

  • You must meet the following income guidelines:

How Do I Apply?


Any of the GAMP contracted community clinics can complete an application which will be forwarded to the GAMP office. A decision is rendered within 15 business days of receipt of the application.



Where Do I Apply?


The following clinics have a GAMP Health Care Specialist on site who can determine the eligibility on the date of application.

In addition, all hospital emergency departments in Milwaukee County may take GAMP applications for patients seen there.

  • Fertility/Abortion/Family Planning Services
  • Transplants - Mostly covered by T-19
  • Mental Health/Substance Abuse
  • Administrative Services
  • General Vision/Eyeglasses
  • Hearing Aids
  • Dental - Except extractions
  • Cosmetic Surgery
  • Therapeutic Devices or Appliances

Commonly Asked Questions


Q1. I am on GAMP and I have received a bill.

  • Contact the name on the statement to make sure they are aware you are on GAMP.

    NOTE: GAMP does not cover all services.
    GAMP does not cover services outside Milwaukee County,Regardless of Emergency level.
    GAMP does not cover unauthorized emergency room visits.

    To seek EMR services you MUST have prior authorization from the *following:

  • Contact your clinic during office hours

    or

  • Contact the GAMP nurse line, which is available 24/7 to answer your questions: 1-888-833-7913

    * These are the only two valid authorizations.

Q2. I just got approved for GAMP but I can't get prescriptions.

  • There is a 72 hour window for eligibility to reach the pharmacy database.

Q3. How do I get refunded for the meds paid for.

  • You must seek refunds from the pharmacy. GAMP does not give refunds.




What medications does GAMP cover?


Covered Medications

The General Assistance Medical Program covers most medications prescribed by doctors.For more information regarding what drugs are covered, please see the GAMP Formulary.

The following are examples of medications not covered by GAMP.

  • Impotency and Infertility Medications
  • Smoking Deterrents
  • Weight Loss Medications
  • Experimental Medications
What do I need to apply?


  • Form of Identification with photograph
    (Wisconsin Driver's License, passport, school I.D., Milwaukee County I.D.)

  • Proof of Milwaukee County Residency
    (Wisconsin Driver's License, current lease with lessor, Phone number and address, current utility bill {except water bill}, faxed memo from your place of residence {shelter, halfway house}.)

  • Proof of Income
    (Check stubs, statement from your employer, award letters, etc.)

  • Verifiable Social Security Number

  • A Cashier's Check, Money Order or Personal Check
    In the amount of $50.00 made payable to Milwaukee County GA-MP. This is a non-refundable per person administrative fee for processing your GA-MP application.
