The following is a list of forms available for download from the internet. Due to the complexity of certain forms, not all permits, applications or documents are available. One can print out the form and mail it in. BEWARE! You must file the correct number of copies. Check the bottom of the page for the correct amount. If you have questions, call the corresponding section for help. See the DNS PHONE Directory for a listing of all sections. All documents will require ADOBE ACROBAT PDF plug in.
Code Compliance Section
CODE COMPLIANCE BROCHURE (DNS-160).pdf-Identifies areas, provides program requirements, exceptions and procedures. (WARNING: This is a legal size document.) If printing, your printer will require legal size paper.
CODE COMPLIANCE APPLICATION (DNS-161).pdf-Application form for Code Compliance inspection and certificate.
Commerical Enforcement
HOME OCCUPATION LICENSE APPLICATION (DNS-164).pdf- This form is for those who are going to use their home as a base of business operations or will be conducting business in the premises. If the premises are rented or leased, the the business operator needs to file an Owner Consent Form granting the leasee the right to use the property for business. (Junk Dealers/Collectors operating out of their home should see the Junk Collectors rules and regulation brochure in the (BROCHURE) section of the DNS web site.

Environmental and Nuisance Section
NOISE VARIANCE APPLICATION FORM.pdf- This form is for those instances were a known event will generate loud noise (i.e. festival, or temporary event) It is to be applied for before the event and requires Aldermanic and DNS approval.
Property Recording
PROPERTY RECORDING APPLICATION (DNS-8).pdf-Used to record a property with the department. Filled out by the new owner. (Note:This is a legal size document.)
PROPERTY RECORDING INSTRUCTIONS (DNS-8Instruct).pdf-Detailed line-by-line instructions for filling out the Property Recording Application.
SELLER NOTIFICATION-Previous owner uses to inform department of the sale of a property and identifies the new owner.
ADD PROPERTIES-A form for owners to add a list of additional properties to their name on one application.
ADD OWNERS- A form to add a list of owners to a property recording application.
OPERATOR RESIGNATION (DNS-8E).pdf-Used to notify department that operator is no longer in control of property. (Relieves operator from responsibility.)
CHANGE WATER BILL ADDRESS.pdf-Form to use to alert the water utility about a new place to send the water bill.
TITLE SEARCH FORM.pdf-Use this form to request a search of Department of Neighborhood Services records for Pending Special Charges and Outstanding Code Violations. Other items included in the search are Code Compliance, Property Recording, Rent Withholding and Fire Inspection. There is a $35.00 fee per address for this search. The property owner will be responsible for any special charges assessed on the property tax bill where a Title Search was not requested in writing prior to the conveyance.
