The State of Wisconsin requires all children attending child care facilities be fully immunized to reduce the chance of disease outbreaks.
In addition, the State requires all child care providers to document and report each child's immunization history. (Group centers HFS 46.04, Family centers HFS 45.04)

Centers that fail to comply with the state regulations will be cited for non-compliance.
Childcare centers are then required to post CFS 294 Non-compliance Statement & Correction Plan until the next CFS-294 or CFS-785 Compliance Statement is issued.
The following enforcement actions will be taken if childcare centers fail to comply: • forfeiture, • notice of revocation, • non-renewal, • summary suspension, • and/ or closing of intake of license.
