Certification Services for New and 5 Year Update Certifications

Certification is the process used to determine the eligibility of businesses that are interested in disadvantaged business opportunity programs. Becoming a certified DBE may create opportunities for small businesses to perform work with Milwaukee County.

If you are a new Unified Certification Program (UCP) applicant, you will need each of the following documents to prepare your application.

• Instructions for completing the application
• The UCP Application
• The Personal Financial Statement (PFS) form
• Document checklist

If you are a returning UCP applicant applying for a 5-year update of your certification, you will need each of the following documents to prepare your application.

• The UCP 5-Year Update form
• The Personal Financial Statement (PFS) form
