Plan Changes for 2009

Changes made during 2009 Open Enrollment became effective January 1, 2009.

Logon to
www.benefitenroll.com to view your 2009 benefit elections. You may review your benefits any time during the year (click the link "Review Employee Benefits") or request changes to your benefits through a qualifying Life Status Event (click the link "Change Benefits). If you need help using the system, contact your local HR or Payroll clerk who has been trained to assist you.

Your health plan benefits for 2009 are not changing. However, UnitedHealthCare will
 administer Milwaukee County's health insurance plans.

  • Milwaukee County Choice Plan (HMO comparable) offers the following benefits as long as you stay in your provider Networks. Highlights for the planinclude:
    • Physician office visit $10 co-pay
    • Paid-in-full immunizations and injections
    • Paid-in-full well-baby care
    • All benefits provided by in-network providers only
  • Milwaukee County Choice Plus Plan (PPO comparable) features comprehensive benefits with deductibles and coinsurance at both in and out-of-network providers. Highlights of the plan include:
    • $150 per person, per year deductible ($450 per family)
    • In-network physician office visit $20 co-pay
    • Paid-in-full well-baby care
    • In-network and out-of-network benefits

 

The prescription drug program will be administered by Medco. The Medco formulary can be found at www.medco.com.

All employees are eligible for the Employee Assistance Program (EAP). Benefits for employees and dependents are provided by MHN/HMC. Additional information may be accessed at
www.mhn.com

All employees are eligible for a discounted Milwaukee County Transit System bus pass for $10/month. Enroll in this benefit at www.benefitenroll.com. Instructions for enrolling are on the home page of the enrollment system.
